To follow are a number of questionnaires for children who are in Pre-School, Kindergarten, or Child Care which need to be completed prior to coming to the first appointment for your child. There are two questionnaires for parents/carers and one questionnaire for your child’s teacher which can be printed, faxed or emailed. These questionnaires are a useful and effective tool which are important to have at your first session.
